After booking a private consultation (see contact details below), you’ll be sent a comprehensive questionnaire to fill in, this will include details of your medical history, symptoms, diet, lifestyle etc. The results will then be discussed during your consultation, when a personalised diet plan and supplement regime will be devised for you. Depending upon your condition, follow-up consultations may be recommended.

As with the private consultation, you’ll be asked to fill in a questionnaire.

After you’ve completed and submitted the questionnaire, you’ll receive your personalised diet plan and supplement regime via e-mail (please note, this may take up to 48 hours). Follow-up consultations may be recommended, if so, you can complete the follow-up questionnaire by clicking here. Once this has been submitted you will again receive your updated diet plan via e-mail.

After contacting holistix – either by telephone or e-mail (see contact details below), you’ll be sent the holistix questionnaire to fill in. Once completed and returned (you may be contacted by telephone for any additional information needed), a diet plan and supplement regime will be devised for you and sent to you by post. Follow-up consultations may be recommended. If so, this again can be arranged by post.
